Special Health Care Needs

The Star Program

Medical Home Initiative with a “Star” program for children with special health care needs.

In 2001, in an attempt to improve patient/family centered care, communication, coordination of

care and monitoring of children with special health care needs, Reading Pediatrics established the

Star Program.

As part of this program, ‘Star’ will highlight your child’s name, and signify to all members of Reading

Pediatrics that you and your child deserve special recognition. This designation alerts our staff to

ask about any special needs – such as urgent scheduling of appointments, extended time with your

provider, contact with our clinical care coordinator and more.

Special Health Care Needs Can Frequently Change

Because of the many special health care needs your child may have, we understand that treatments, medications and other important information may change frequently. With that in mind, we will ask you to review the current information we have on file and make any necessary changes each time your child is scheduled for an appointment in our office. We feel this process will provide invaluable information to those providing care for your child.

Please visit Facebook and consider ‘liking’ us on our Reading Pediatrics Star Program page. We communicate interesting and valuable information for families of special needs children.

Please feel free to call with any questions that may arise. We have a dedicated mailbox to provide for these calls - ext. 3150. As always, we strive to provide the best possible healthcare for you and your child.
